Condition Management

We take a whole-person approach to managing medical conditions such as heart failure, COPD, asthma, coronary artery disease and diabetes.

Help people take ownership of their health

Our condition management programs identify at-risk individuals and help resolve health care gaps by providing the right resources at the right time. Ninety percent of participants felt the program helped create a better, more positive health care experience.

Realize cost savings with personalized care

Dedicated nurses create strong relationships and deliver personalized care to improve outcomes and mitigate costs. A typical range of savings is $0.90–$4 per person per month.

Support health with remote monitoring

Remote monitoring is available for the heart failure and COPD programs. The individual weighs in twice a day and responds to questions about symptoms. Information is sent by phone to the nurse, who reviews it and responds proactively.

That’s everything I want to hear… medical information, hear it as relatable to my needs and identify the right path or paths to get better.

Encourage engagement in the right program

Data and analytics allow holistic viewing and monitoring of total population health. This helps us identify individuals closer to the time they experience a change in health, when they are more likely to act and engage.

Provide meaningful support, trusted guidance

Nurses are at the center of our programs. They listen and understand, and help members feel engaged in their own health. With access to integrated technology, our nurses collaborate with the individual’s health care team to help him or her better manage conditions.
