If you don’t see the record you need
Some records, including older ProHEALTH or CareMount records, may not appear in MyChart.
If you’re unable to locate a specific record in the portal, or if you’re making a request on behalf of a patient, you can request medical records using other options.
Who can submit a request:
- Patients who can’t find a record in MyChart
- Parents or legal guardians for patients under 18 or those unable to give consent
- Authorized personal representatives
- Health care providers requesting records for treatment purposes
Important notes:
- Parents/legal guardians must sign requests for minors or patients unable to provide consent.
- Patients may designate a personal representative in advance.
- Providers should include facility name, provider NPI and a secure fax or delivery method.

You may also request a copy of your medical records by completing a paper request form.
- You can download printable forms in English and Spanish.
- Forms submitted with missing or unreadable information, insufficient legal authority or an incorrect signature may result in processing delays
- If you are asking for records for another individual, you must provide documentation showing legal authority.
