CareMount Value Partners IPA is a realizing equity, access and community health accountable care organization (REACH ACO) and a leading provider of value-based care. 

How a REACH ACO can help you

A REACH ACO is a group of doctors, hospitals and other health care providers who agree to work together to provide you with high-quality care in a way that lowers overall health care costs.

For all our members who are Medicare patients, this will not impact your benefits. In fact, it will only add to the care you receive.

Our ACO is made up of primary care providers and specialists who have agreed to work together with us to coordinate care.

These include home health agencies and skilled nursing facilities/centers.

Note: Medicare fee-for-service beneficiaries have the freedom to seek the services and providers of their choice and are not limited to the providers listed above.

There are no joint ventures between or among the CareMount ACO and any of its ACO preferred providers and participant providers.
