Filling up the short-term disability insurance paperwork for my employer, as requested by my employer was not done this time. It is a legal matter for my employer, and for myself I believe. I'm not Certain why it is so difficult to get the paperwork for FMLA and this type of documentation for short-term disability insurance filled out. My whole life hangs on the balance

I am very disappointed because of my prescriptions. The doctor was unable to successfully send my GLP one prescription to the dynamic RX Successfully, he left out one step a B12 addition to the Prescription . A new pharmacy that I requested for my GLP one to be sent to. He also sent my medication's to that pharmacy so then that messed up the process of my medication's being filled at my pharmacy that I usually go to for all of my Local Pick Up medications. I'm just overall disappointed in [omitted] care because I keep getting a new doctor every eight months. Seems like they can't keep staff at the facility.
