How to order DME equipment and supplies
The process for getting DME equipment or supplies starts with your health care provider. Here’s what you can expect:
- Your doctor determines if you need certain services or equipment, then writes a prescription or order for it.
- Your insurance coverage will be verified and prior authorization obtained.
- Claims will be submitted to your insurance company or Medicare.
- Southwest Medical Pharmacy and Home Medical Equipment will work with you and your doctor to arrange for delivery, usually to your home.

Diabetic testing supplies delivered to your door
Optum Nevada members can receive glucose testing supplies shipped directly to your home.
Getting started is easy:
- Ask your doctor to fax a prescription to Southwest Medical Pharmacy & Home Medical Equipment at 1-702-242-7703.
- After you’re an established patient, call 1-702-796-1016 for support, Monday–Friday, 8 a.m.–5 p.m. PT.
