Request records using a paper form
If you prefer not to request medical records online, or if you’re an authorized representative requesting records for a patient, you can send us a completed release of information form in English or Spanish.
Forms must be filled out correctly. Forms with missing or unreadable information, insufficient legal authority or that are signed by the wrong individual may delay processing.
Also, if you are an authorized representative by law for a patient, you need to provide documentation that shows this when submitting the completed form.
Optum may charge a reasonable, cost-based fee for a copy of your PHI, as permitted by the HIPAA Privacy Rule and state law.
