How HouseCalls helps address social drivers of health through in-home clinical care

HouseCalls nurse practitioners see Medicare Advantage members in their homes, allowing them to identify critical social drivers of health that may go unnoticed in traditional clinical settings, like housing instability, lack of utilities and unsafe living environments.
Ashley, a HouseCalls nurse practitioner in Louisiana, saw this firsthand. During a HouseCalls visit, she found her patient living in a partially burned home with no electricity or running water. These unsafe conditions were directly impacting the patient's health, yet they may not have been identified during a typical doctor's office visit.
Thanks to HouseCalls, Ashley connected the patient with local resources to secure safe housing — one of over 553,000 referrals made through the program to address social drivers of health.
