There is a disconnect between the intake of the nurse, the questionnaires, and what the doctor sees. I had a list of things that I communicated...some were communicated to the nurse, some through the webportal when making the appointment, and some on a questionnaire. I forgot to consult my list, so a few items were not discussed. So, it seems like there are too many ways to communicate what is going on, and so then things fall through the cracks.
