I went in for FMLA paperwork was the first time I had visit this provider. she looks nice and presentive herself very personable but at the end she fills my FMLA paperwork wrong. like she was not listening to me. She put on that paper to TBD and no yearly date on them. and of course, my FMLA was denied. I show her if she wanted to see my last paperwork she told me she knows how to fill them up big ego. Then I return those the paperwork on Monday for her to revised she never call me to give paperwork back I had to make another appointment with Dr warren a very nice person to do again. I'm an establish patient there or more than 5years. A bit disappointed.
