Norman stated he had completed reasonable accommodation paperwork the Friday before my appointment and had sent it back to my employer. My employer still has not received a paperwork. I initially submitted the paperwork 5 to 6 weeks ago. Also solve the provider he stated he would send a referral to the orthopedic provider due to a right bicep tear. That was nine days ago and the provider still has not sent a referral. I’ve had repeated problems with the spine staff, not completing paperwork, timely, or not being very friendly when I call to get updates on the paperwork. At this point, I’m gonna reach out to the group practice manager for help. In my opinion, this has resulted in a significant delay of care.

Norman is so professional, friendly and easy to talk with about my pain in my back. He asked lots of questions and paid close attention to my words.
