A new approach to risk adjustment

Prospective Solutions provides a comprehensive set of services that support whole-person care via multiple care modalities. By using predictive analytics, members are steered toward the modality that they prefer, such as in-office, in-home or telehealth visits. 

The Optum field team follows up with members to help schedule referrals, identify care barriers and offer additional resources. The field team works closely with providers to ensure office staff receive admin support and provide coding and documentation training.

Through personalized member interventions, end-to-end solutions and improved care coordination, Prospective Solutions delivers a new approach to risk adjustment.  Optum offers the scale and results payers and providers need to help deliver better outcomes for members.
