SF-10 Health Survey for Children™
Intended for children age five to 18, the SF-10™ Health Survey for Children contains 10 questions adapted from the Child Health Questionnaire (CHQ). It provides coverage across a wide range of domains. It is scored to produce physical and psychosocial health summary measures.
Due to its brevity, this parent-completed survey can be easily integrated and administered within a broader assessment. It is particularly applicable to large-scale child population surveys and is available with a standard four-week recall period in multiple languages.
Pediatric Asthma Impact Survey (PAIS-6)™
The Pediatric Asthma Impact Survey™ (PAIS-6™) is a six-question, parent-completed survey measuring the impact of asthma on a child's functional health and well-being.
The PAIS-6™ covers a wide range of asthma impact, complementing traditional measures of disability associated with asthma. This survey is intended for children between the ages of five and 17 and is available in U.S. English with a standard four-week recall period.
