The HouseCalls™ program provides an annual in-home clinical preventive assessment for members enrolled in qualified health plans. 

Annual visits — within patient homes

An annual visit includes a physical exam, as well as a comprehensive review of the member’s health history and medications. Some preventive screenings are also available, all without stepping foot inside a doctor’s office.

Coordination with member's doctor

All visit results are reported back to the member’s primary care provider (PCP), fostering communication of care plans. This also helps to facilitate referrals to specialty care as needed.

Home and health assessment

In an effort to help make sure members keep their freedom and live safely in their homes, clinicians take the time to identify health risks during each visit. This includes assessing the member’s home environment and providing health education where needed.
