For Project year 2017, there have been over 200,000 quality gaps in care closed from a total of 400,484 gaps submitted, across different measures.
Quality Gaps Closed (Data as of July 31, 2017)
- Over 74,000 quality gaps closed (93%) for the Adult BMI Assessment measure, from a total of 79,397 gaps.
- Over 43,000 quality gaps closed (80%) for the Controlling Blood Pressure measure, from a total of 53,194 gaps.
- Over 14,000 quality gaps closed (39%) for the Blood Sugar Level (HbA1c) Screening measure, from a total of 36,083 gaps.
- Over 11,000 quality gaps closed (37%) for the Care of Older Adults Pain Screening and Care of Older Adults Medication Review measures, from a total of 29,404 gaps.
- Over 7,000 quality gaps closed (46%) for the Care of Older Adults Functional Status Assessment measure, from a total of 14,909 gaps.
- Over 5,000 quality gaps closed (14%) for the Diabetic Nephropathy Screening measure, from a total of 34,399 gaps.
- And between 1,000 and 2,000 quality gaps closed
(1-2%) for each of the Breast Cancer Screening, Diabetic Eye Exam, and Colorectal Cancer Screening measures, from a total of 98,996 gaps.
Please continue to ensure that medical record submissions include all data elements required per measure in order to continue closing gaps in care.

We no longer offer SFTP as an option to submit your Healthcare Quality Patient Assessment Forms/ Patient Assessment Forms (HQPAF/PAF). We encourage you to use our new HQPAF/PAF Uploader; an easier and faster submission method. The HQPAF/PAF Uploader requires less time and effort than preparing and submitting documentation via mail or fax.
The HQPAF/PAF Uploader was developed to provide an interface that allows multiple users to securely submit HQPAFs/PAFs without issue of failed delivery or suspended access. The HQPAF/PAF Uploader:
- Does not require any user credentials
- Complies with all HIPAA guidelines to protect your practice and your patient’s personal data
- Allows anyone in your office to access the site and upload documentation
- Securely transmits directly to Optum
