Receiving your 2015 PAFs/HQPAFs

Welcome to the 2015 Patient Assessment Form/Health Quality Patient Assessment Form (PAF/HQPAF) program. The goal of the PAF/HQPAF program is to help ensure that all of your Medicare Advantage Plan patients are seen in the office at least once per year, and that all of their chronic conditions are documented and coded to the highest level of specificity. The PAFs/HQPAFs can help guide the provider’s assessment of each eligible patient to help close quality and risk adjustment gaps in care.

If you have not already received your 2015 PAFs/HQPAFs, you should receive them very soon. You can also contact your Healthcare Advocate to ensure timely delivery of your 2015 forms. If your practice has utilized the OptumPAF™ portal in 2014, your 2015 forms are now available in the portal’s “Inbox” upon log in.

Once you have your PAFs/HQPAFs, it is important to ensure that all of the patients in your practice’s PAF/HQPAF profile are scheduled for an appointment in 2015. The PAF/HQPAF program was designed to be a prospective tool that can be completed at the time of the patient’s encounter with the provider. Once completed, a progress note must be attached to each PAF/HQPAF prior to submission. PAFs/HQPAFs deployed on paper can be submitted via secure fax or traceable carrier, while PAFs/HQPAFs that are deployed electronically can be submitted via the OptumPAF portal or using the SFTP (secure file transfer protocol) method.

Regardless of how PAFs/HQPAFs are submitted, it is important that you work with your Healthcare Advocate to ensure quality PAFs/HQPAFs prior to submission to minimize rejections and ensure gap closures. Your Healthcare Advocate can provide you with various tools and education that will ensure PAF/HQPAF success.
