LVHN takes a new approach

The January 2018 issue of the AMGA Group Practice Journal highlights LVHN's multi-strategy approach.

Bringing together patient data sources needed to successfully prepare and execute data-driven population strategies is a challenge for most health care organizations. A lack of interoperability when exchanging and using information across health systems adds to the problem.

Lehigh Valley Health Network (LVHN), an Optum Analytics client, created a viable solution to solve their interoperability challenges. Their solution combined risk stratification and aggressive interventions with the transmission of crucial point-of-care data between patient registries and EHR.
