Rising to the COVID-19 challenges together

Optum and Gulf Coast Health Care have partnered for five years to deliver clinical care and benefits to members in long-term skilled nursing facilities. Through their collaborative efforts, Gulf Coast reduced member hospitalizations and ED visits, improved ratings and delivered shared savings.

Gulf Coast was in the process of introducing the Optum care model to five more facilities when COVID-19 struck. Fortunately, their relationship with Optum laid the foundation of trust and experience for them to work together.

Optum and Gulf Coast collaborated to ensure they implemented the care model in half the time it normally takes. They also achieved the following:

  • Set up a digital platform to help ensure easy contact between Optum and facility members, families and staff.
  • An on-site advanced registered nurse practitioner helped physicians address any member needs, reducing the need for hospitalization.
  • An on-site advanced practice clinician (APC) acted as an extra set of eyes and hands for care teams, physicians and members’ families.
  • The APC ensured members had access to on-site flu shots, on-site medical visits, hearing aids and qualifying medical supplies and equipment.
